This all-in-one form allows you to disconnect service, make name changes to your account, sign-up for bank draft, paperless billing, request a hardship, request for your account information to be confidential and/or a bill adjustment. Bill adjustments include penalty waivers or a leak adjustment. Leak adjustments are for residential customers only and are applicable once per year with a maximum consideration of two bills adjusted. Using this new form, you can select the type of request you need and submit in one place.
